Strengthening Team-Based Care to Enhance Transplant Outcomes

To continuously enhance the quality of clinical care in organ transplantation, the Organ Procurement and Transplantation Center of Shuang Ho Hospital held the “Symposium on Heart Failure and Transplantation” on March 21, 2026. Through interdisciplinary integration and practical exchange, the program aimed to strengthen the expertise of healthcare teams in transplantation medicine while reinforcing a patient-centered, holistic care approach.The course covered the latest clinical concepts in heart failure and heart transplantation, offering comprehensive and systematic training across the full spectrum of care—from patient evaluation and organ matching to surgical risk management and post-transplant care. Special emphasis was placed on the importance of multidisciplinary collaboration, bringing together physicians, nurses, pharmacists, and rehabilitation professionals to facilitate knowledge integration and experience sharing across disciplines.From a pharmaceutical perspective, pharmacists provided in-depth analysis of drug mechanisms and clinical applications in transplantation, enabling the healthcare team to optimize medication strategies and risk management. Meanwhile, rehabilitation specialists highlighted the critical timing and impact of interventions on postoperative functional recovery and quality of life, underscoring the essential role of rehabilitation in the transplant continuum. These diverse professional insights allowed participants to develop a more comprehensive and in-depth understanding of overall treatment strategies.Organ transplantation is a major milestone in modern medicine, significantly extending patient survival while greatly improving quality of life. However, its clinical practice involves highly complex evaluation and decision-making processes, requiring close collaboration among multidisciplinary teams to ensure healthcare quality and patient safety. This program also emphasized post-transplant care and complication prevention, strengthening clinical competencies and further improving transplant success rates.Participants included transplant-related physicians (from surgical, medical, and critical care specialties), transplant nurse specialists, emergency and intensive care nurses, social workers, mental health professionals, and transplant coordinators, demonstrating the hospital’s strong commitment to interdisciplinary collaboration.Looking ahead, the hospital will continue to promote integrated, multidisciplinary education and clinical exchange, enhance the professional capabilities of healthcare teams, and strive to deliver safer, higher-quality, and compassionate transplant care—bringing greater hope and improved quality of life to patients.
